In Moscow, the process of continuing treatment after hospitalization has been simplified for patients who have suffered a heart attack or stroke. This was reported by Anastasia Rakova, Deputy Mayor of Moscow for Social Development.

“Moscow is consistently developing assistance for patients with cardiovascular diseases. Thanks to the digitalization of medical processes, the large volume of data, and management tools, we have observed that not all patients start the necessary drug therapy within the first month after discharge following a heart attack or stroke. At the same time, this period is especially important for further recovery and prevention of recurrent heart attacks and strokes. Therefore, we have changed our approach, and now the patient receives the first supply of subsidized medications for 30 days directly upon discharge from the hospital. This allows for the immediate start of essential treatment and increases the likelihood that the patient will adhere to the prescribed therapy,” said Anastasia Rakova.

Patients after heart attack and stroke, who are entitled to the corresponding benefit, receive the necessary medications already upon discharge from the hospital. If the benefit was not previously arranged, it is done during hospitalization.

Information about the prescription is also automatically sent to the clinic, the doctor sees the patient and subsequently renews their medication prescription in advance. If necessary, the specialist can contact the person via telemedicine service, inform them about the prescription renewal, and also clarify their condition. If the patient has complaints or feels unwell, they are invited for an in-person appointment.

This approach allows not only to immediately provide the patient with the necessary medications after discharge, but also to make the continuation of the prescribed therapy more consistent and convenient.

Work on improving medical care for patients with cardiovascular diseases is being carried out within the framework of the national project “Long and active life”. Its key task is to increase healthy life expectancy and reduce mortality from major chronic diseases. In Moscow, within the framework of the project, modern approaches to the prevention, diagnosis, and treatment of cardiovascular diseases continue to be developed, as well as improving access to medical care at all stages — from early detection of risk factors to recovery after suffered diseases.
