In Moscow, a voice assistant based on artificial intelligence (AI) has become available to all district doctors of adult city clinics. The service transcribes the conversation between the doctor and patient in the background and automatically fills in the necessary fields of the reception protocol. The solution helps specialists spend more time communicating with the person. This was reported by Anastasia Rakova, Deputy Mayor of Moscow for Social Development.
“In Moscow, we are implementing a new model of work for the district physician, which involves continuous patient support and responsibility not only for the quality of individual appointments but also for the overall treatment outcome and health condition. To achieve this, we employ the best global practices and consistently develop digital solutions that become a reliable “co-pilot” for the physician, supporting them at various stages of working with the patient. One such solution is the automatic filling of appointment protocols using an AI assistant — Moscow has implemented it for all district physicians. It transcribes the conversation between the doctor and the patient in the background and adds information about complaints, medical history, diagnoses, recommendations, and other details mentioned during the appointment into the protocol. Today, this service operates with all district physicians of adult city polyclinics in Moscow — more than three thousand specialists. This is the result of significant collaborative work between developers and doctors: their feedback over several months helped us achieve stable service quality,” said Anastasia Rakova.
In the summer, the voice assistant aided during appointments with general practitioners in a test mode. This is a particularly challenging task for artificial intelligence: people come with a wide range of complaints, often with multiple diseases and a long history of treatment. The service must accurately recognize the conversation, identify significant clinical information, and correctly allocate it to sections of the protocol. Therefore, refining the technology took time and required constant involvement of specialists. General practitioners shared feedback, pointed out errors, and helped evaluate the changes made in real appointment conditions.
The implementation of the service became part of the new working model for the district doctor. It involves long-term support for the patients in their area. The specialist knows their health history, forms a comprehensive understanding of each patient’s condition, coordinates diagnostics and treatment, interacts with specialized doctors, and monitors the results.
The service is already in use by all neurologists at adult city clinics. The first results of its implementation showed that the time of focused attention of the doctor on the patient increased more than twofold. Positive changes were also noted by patients: they rated the specialist’s attention higher, found his explanations clearer, and began to trust the prescribed treatment more. Experts rated the quality of the reports prepared by the AI service at 4.8 out of five possible points.
The digital assistant automatically enters complaints, medical history, diagnosis, recommendations, and other information mentioned during the appointment into the report. At the end of the consultation, the doctor reviews the prepared document, makes corrections if necessary, and signs it. The system has been trained on anonymized data from real appointments, which helps it recognize medical terminology. All information, including transcriptions, is transmitted through secure channels of the unified medical information and analytical system.
Smart reception services operate in capital city polyclinics. This is the most important element of the new model for organizing outpatient care in Moscow, whose main task is to make the appointment as effective as possible for both the patient and the doctor. Smart services are intended to reduce the number of visits that do not require a doctor’s examination and save time for specialists and patients. In addition, they help detect diseases at an early stage: artificial intelligence analyzes responses during registration, data electronic medical record and assesses the risks of diseases.
Moscow has been engaged in the digitalization of the healthcare system for more than 10 years. The foundation of this process has been a unified digital platform, which is being developed by the capital’s Department of Health and Department of Information Technology. Moscow supports the philosophy of healthy longevity: digital solutions help continuously accompany a person — from the first complaint and diagnosis to treatment, evaluation of its results, and subsequent monitoring.
