Doctors at the V.M. Buyanov City Clinical Hospital (CCL) saved a 42-year-old man after a massive heart attack, which led to multiple cardiac arrests. This was reported by Anastasia Rakova, Deputy Mayor of Moscow for Social Development.

“In Moscow, an emergency medical care system has been established that allows treatment to begin in the first hours of disease development. Thanks to the infarction network, evenly distributed throughout the city, patients with heart attacks undergo emergency operations in the shortest possible time, which allows saving lives even in the most severe cases. Specialists promptly conduct diagnostics, perform complex interventions, and choose an individual strategy considering the dynamics of each patient. In the case of a 42-year-old man, the doctors at the V.M. Buyanov hospital made decisions at each stage based on his condition, complicated by an extensive infarction. After emergency stenting and intensive therapy, positive progress appeared. However, after three days there was a sharp deterioration, and the patient suffered multiple cardiac arrests. The doctors immediately began resuscitation and used ECMO—a system that temporarily takes over some functions of the heart and lungs, supporting circulation and oxygen saturation of the blood.””Thanks to the competent actions of the specialists, the individualized treatment plan, and the capabilities of modern medical equipment, the patient’s condition was stabilized, and he was discharged from the hospital after just 10 days,” noted Anastasia Rakova.

The patient was admitted to the vascular center at V.M. Buyanov City Clinical Hospital with a sharp pain behind the sternum. According to him, the sensations were so intense that his arms went numb. The man managed to call an ambulance and leave the apartment door open, after which he lost consciousness. The medical team urgently transported him to the hospital.

The examination revealed an extensive myocardial infarction and almost complete blockage of the anterior interventricular artery by a thrombus — one of the major vessels supplying blood to the heart muscle. To restore arterial blood flow, specialists performed balloon angioplasty and placed a stent. The extensive heart damage caused cardiogenic shock — a condition in which the organ cannot effectively pump blood and supply the organs with the necessary amount of oxygen. In addition, ventricular fibrillation occurred — a dangerous cardiac rhythm disturbance characterized by the absence of coordinated contractions. The doctors performed defibrillation and started intensive therapy.

Three days later, the man’s condition sharply worsened, and several cardiac arrests occurred. During resuscitation measures, specialists identified thrombosis of a recently implanted stent caused by individual characteristics of the body and eliminated it. Due to the ineffectiveness of standard treatment methods, the patient was transferred to ECMO. The man’s condition was continuously monitored, tracking the functioning of the heart and all body systems. After three days of support, cardiac activity was restored, and the ECMO procedure was completed.

After the condition stabilized, doctors began early rehabilitation. It was conducted while he was still in the intensive care unit. Initially, the man performed passive gymnastics, then started sitting up, standing, and walking independently. An individualized diet was selected for him, taking into account the state of his body and the stage of recovery. Ten days after admission, the patient was discharged and today continues to undergo rehabilitation, as well as take the necessary medications that were prescribed and provided to him in the hospital for the first month of treatment.

The healthcare system in the capital is developing within the framework of the national project “A long and active life”. A new standard of medical care has been introduced, providing for an effective routing system, the purchase of modern equipment, the use of advanced diagnostic and treatment methods, comprehensive rehabilitation and prevention, as well as the provision of free medications. This allows for timely necessary assistance, reduces the risks of complications, and creates conditions for maintaining health and improving the quality of life of Muscovites.
